Recommendation 1

1. The service should ensure that care plans and risk assessments are accurate, analytical, and SMART and are responsive to changes in young people’s circumstances providing clear strategies to address these. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS), which state that: ‘My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met, as well as my wishes and choices.' (HSCS 1.15)

Recommendation 2

2. The service provider should ensure that the matching process outlines clearly the basis for decisions being made. There should be a detailed impact assessment for each young person affected by the transition highlighting the positive aspects of any change and any potential risks that might undermine emotional wellbeing or physical safety. There should also be robust matching of how staff skills and experience will be suited to meet the needs of the young people. As highlighted in: Matching Looked After Children and Young People: Admissions Guidance for Residential Services; January 2024. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'My care and support meets my needs and is right for me’ (HSCS 1.19).
