1. The provider should ensure that each person has a personal plan and risk assessments in place that accurately reflect their current, individual care and support needs. This should include ensuring that: a) care plan documentation is sufficiently detailed, outcome focused, and written in a person-centred manner b) all care plans and risk assessments contain accurate and corresponding information, including information relating to support with medication c) care plans and risk assessments are updated whenever a person’s needs or circumstances change and regularly audited. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: ‘My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met, as well as my wishes and choices’ (HSCS 1.15).
