1. To provide a care and support service to adults with complex support needs living in their own home and the wider community.
2. The adults using the service will be living at an address agreed at registration. The Care Inspectorate must be informed of any subsequent changes.

3. The support will be provided by one staff team located at the addresses agreed at Registration. The Care Inspectorate should be informed of any subsequent changes.
4. The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details, as it appears on this certificate.
