1. The provider should ensure that quality assurance and improvement processes lead to clear, outcome focused actions which are consistently recorded, monitored, and evaluated through the service improvement plan, including the use of SMART approaches to demonstrate progress and impact. This is to ensure care and support is consistent with the Health and Social Care Standards (HSCS), which state that people should benefit from “a culture of continuous improvement, with robust and transparent quality assurance processes” (HSCS 4.19)
1. The provider should develop care related documentation by ensuring: Daily recording is person-centred, clear and readable and personal plans clearly set out people’s expressed outcomes and aspirations to support consistent, meaningful support. This is to ensure care and support is consistent with the Health and Social Care Standards (HSCS), which state: “My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met, as well as my wishes and choices” (HSCS 1.15).
Conditions of registration

1. To provide a service to adults with complex support needs living in their own home. This Housing Support Service is also available to adults in the wider community.
2. The adults using the service will be living at an address agreed at registration. The Care Inspectorate must be informed of any subsequent changes.

3. The support will be provided by one staff team located at the addresses agreed at Registration. The Care Inspectorate should be informed of any subsequent changes.
4. The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details, as it appears on this certificate.
