To provide a service to children adults and older people with assessed support needs living in their own homes and in the wider community.
Where the support is provided from more than one address the service must keep the Care Inspectorate informed of any changes to the addresses from which the service is provided.
The service will be provided by one team based in Glasgow to Greater Glasgow and Clyde Lanarkshire Falkirk West Lothian Renfrewshire and surrounding areas.
