1. To provide a service to people with learning disabilities, physical disabilities and mental health conditions living in their own homes and in the community.

2. The support will be provided by one staff team as agreed during this registration and where there are any subsequent changes the Care Inspectorate should be informed.
3. The Registered manager of this service will also manage Longcroft Care Home CS2011303445.
4. The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details, as it appears on this certificate.
