1. The service should make improvements to oversight of covert medication pathways to ensure they remain accurate and reflective of individual's current needs and capacity. Documentation should be regularly reviewed in line with best practice guidance, with timely action taken where reviews are overdue or documentation requires updating. This is to ensure care and support is consistent with the Health and Social Care Standards (HSCS) which state: "If I need help with medication, I am able to have as much control pas possible." (HSCS 2.23).
1. To ensure people consistently receive prompt support when they need it, the service should improve how it monitors and evaluates staff response times to the nurse call alert system. This should form part of the overall staffing assessment. This is to ensure care and support is consistent with the Health and Social Care Standards (HSCS) which state: "I am confident that people respond promptly, including when I ask for help" (HSCS 3.17)

To ensure people's needs and preferences are met and people experience good outcomes, the provider should develop personal plans to include life histories and ensure they reflect people's background, values and what is important to them. This is to ensure care and support is in line with the Health and Social Care Standards (HSCS) which state: "My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met, as well as my wishes and choices" (HSCS 1.15)
