1. To support effective scrutiny of the service, the provider should ensure that managers submit notifications of significant events in accordance with Care Inspectorate guidance ‘Children and young people’s care services: Guidance on records you must keep and notifications you must make (March 2025)'. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'I experience high quality care and support based on relevant evidence, guidance and best practice' (HSCS 4.11).

2. To support young people’s needs consistently through fully risk assessed practice, the service should learn and reflect from incidents of challenging behaviour. This should include but is not limited to, structured debriefs and managerial auditing of incident records. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'I benefit from a culture of continuous improvement, with the organisation having robust and transparent quality assurance processes' (HSCS 4.19); and 'I experience high quality care and support because people have the necessary information and resources' (HSCS 4.27).

3. To support young people to receive care and support that is responsive to their changing needs, the service should ensure that Positive Behaviour Support plans are reviewed within agreed timescales and updated to accurately reflect young people's current presentation, needs, risks and support strategies. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'My personal plan is regularly reviewed and updated, and any changes are discussed with me and reflected in the way my care and support is provided' (HSCS 1.16).
