1. To ensure positive outcomes for people who use this service the provider should ensure; - that daily recordings of care provided should be developed to be more person-centred, detailed and evaluative. - that care documentation reflects the views of people receiving care and support and how they have been involved. - that minutes of reviews reflect the individuals' views even if they choose not to attend formal review meetings. This is to ensure care and support is consistent with the Health and Social Care Standards (HSCS), which state that: ‘My views will always be sought and my choices respected, including when I have reduced capacity to fully make my own decisions’ (HSCS 2.11) and 'I experience high quality care and support based on relevant evidence, guidance and best practice' (HSCS 4.11).
