Recommendation 1

1. In order to ensure that all staff are aware of external health input managers should ensure that any contacts, arranged visits and decisions made by them are clearly recorded. Recommendation 2

1. In order to keep people and staff safe managers should - a) Ensure that people's risk assessments are updated promptly after each review. b) Alert staff to any changes in people's risk assessments. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'My care and support meets my needs and is right for me' (HSCS 1.19).
