1. Each person experiencing care should have their personal plan and risk assessments reviewed and updated to ensure they reflect current assessed needs and risks.
1. The service should ensure all staff feel supported, trained and competent when completing or updating personal plans and risk assessments.
1. To promote people’s wellbeing, the provider should improve its approach to person-centred care planning. This should include, but is not limited to, people’s life histories, likes and dislikes and personal outcomes in the care home. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: "My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met, as well as my wishes and choices" (HSCS 1.15).
