1. To support young people’s wellbeing and promote dignity and respect the provider should review their restrictive practice procedures. This should include but is not limited to the practice of locking doors within the house. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which states that. ‘If my independence, control and choice are restricted, this complies with relevant legislation and any restrictions are justified, kept to a minimum and carried out sensitively.’ (HSCS 1.3) and ‘I am as involved as I can be in agreeing and reviewing any restrictions to my independence, control and choice.’ (HSCS 2.6)

2. The service should ensure that care plans and risk assessments are accurate, analytical, and SMART and are responsive to changes in young people’s circumstances providing clear strategies to address these. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS), which state that: ‘My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met, as well as my wishes and choices.' (HSCS 1.15)

3. The service provider should ensure that the matching process outlines clearly the basis for decisions being made. There should be a detailed impact assessment for each young person affected by the transition highlighting the positive aspects of any change and any potential risks that might undermine emotional wellbeing or physical safety. There should also be robust matching of how staff skills and experience will be suited to meet the needs of the young people. As highlighted in: Matching Looked After Children and Young People: Admissions Guidance for Residential Services; January 2024. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'My care and support meets my needs and is right for me’ (HSCS 1.19).

4. To support children's wellbeing, learning and development, the provider should ensure staff access to training that is appropriate to their role and consistently apply their training to practice and this be reflected in recordings. This should include, but is not limited to, ongoing training in Trauma Informed Practice, ensuring that behaviours are understood and responded to, from a trauma informed perspective. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which states that: ‘I have confidence in people because they are trained, competent and skilled’ (HSCS 3.14).

5. To ensure best outcomes for young people the provider should develop a culture of continuous improvement by implementing robust quality assurance of practice and plans. This should include but is not limited to a) Regular consistent audits of young people's plans and risk assessments to ensure that outcomes are being met and that all risks and their potential impact are reviewed.

6. The provider should notify the Care Inspectorate of incidents as described within the Care Inspectorate publication, "Records that all registered children and young people's services must keep and guidance on notification reporting" REG-0821-067. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'I benefit from different organisations working together and sharing information about me promptly where appropriate, and I understand how my privacy and confidentiality are respected.' (HSCS 4.18).

This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'I benefit from different organisations working together and sharing information about me promptly where appropriate, and I understand how my privacy and confidentiality are respected.' (HSCS 4.18).
