1. To provide a service to people with mental health problems living in their own homes and in shared accommodation and in the community.

2. The support will be provided by 2 staff teams based at the addresses as agreed during this variation and where there are any subsequent changes the Care Inspectorate should be informed.
3. The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details, as it appears on this Certificate.
