Recommendation 1

1. The service provider should ensure care plans are in place which reflect the current needs of the person who uses the service and provide clear guidance on strategies and approaches to be used by staff to meet residents' identified needs. This ensures that support is consistent with the Health and Social Care Standards: 1.15 My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met, as well as my wishes and choices.
