1. By 20th July 2026, the provider must ensure that people experiencing care receive safe, effective and timely support following accidents and falls. To do this the provider must: a) Ensure a clinical assessment is completed immediately following any fall or incident, including injury checks and review of pain and pain management, with escalation to healthcare professionals as required. b) Care plans and falls risk assessments must be updated immediately after a fall or any change in risk. c) The environment must be reviewed following falls to identify and address hazards, including lighting, flooring, doors, call systems, and accessibility. d) Appropriate seating areas must be provided throughout the home to support safe mobility and reduce falls risk. e) All falls prevention equipment and its use must be reviewed regularly, alongside checks of the call bell system, buzzer noise levels, and sta2. This should include: • Consistent continence promotion. • Timely support with personal care. • Monitoring of people’s presentation and comfort. • Improved staff deployment to meet people’s needs. This is to comply with Regulation 4(1)(a) of the Social Care and Social Work Improvement Scotland (Requirements for Care Services) Regulations 2011 (SSI 2011/210). By 20th July 2026, t he provider must ensure there is effective and safe leadership within each unit of the home, supported by robust governance and management oversight. This must include: a) Unit leaders must be appropriately trained, supported, and supervised to enable them to make timely and informed clinical and care decisions, recognise deterioration, and ensure that escalation of care is promptly and effectively implemented when required. b) Unit leaders must be competent and confident to support, supervise, and appropriately delegate tasks to staff, ensuring care is delivered safely, consistently, and in line with people’s assessed needs and care1. In order to achieve this, the provider must: a) Address gaps and inconsistencies in internal maintenance processes, including room checks and water temperature testing, ensuring these are completed regularly and recorded accurately. b) Take immediate action to resolve identified risks related to water temperature monitoring, ensuring all outlets meet required temperature standards to prevent the risk of legionella. To support people who experience stress and distress, the provider should strengthen practice in the anticipation, prevention and management of stress and distress, ensuring staff are competent in recognising early indicators and applying appropriate, personalised interventions. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: "I am confident that the right people are fully informed about my past, including my health and care experience, and any impact this has on me". (HSCS 3.4) "I am supported and cared for sensitively by people who anticipate issues and are aware of and plan for any known vulnerability or frailty". (HSCS 3.18)
1. To improve complaints management processes, the provider should ensure timely investigation, clear responses, shared learning and demonstrable service improvement. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: "I am listened to and taken seriously if I have a concern about the protection and safety of myself or others, with appropriate assessments and referrals made." (HSCS 3.22)
1. To keep people safe and promote their health and wellbeing, communication and recording in relation to health and wellbeing needs should be consistent across the service. This should include but not be limited to professional visits and outcomes logged detailing actions. Communication in relation to people's health and wellbeing should be effective both internally and externally. This is ensure care and support is consistent with the Health and Social Care Standards (HSCS) which state that: "I benefit from different organisations working together and sharing information about me promptly where appropriate, and I understand how my privacy and confidentiality are respected." (4.18) "I experience high quality care and support because people have the necessary information and resources." (4.27)
1. In order that people are able to move around and feel safe and secure in their surroundings, the service should undertake a review of the environment using good practice guidance. This should include but is not limited to: a) easing decision-making and orientation b) encouraging independence and social interaction c) promoting easy access to outdoor space, fresh air and natural light. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'I can independently access the parts of the premises I use and the environment has been designed to promote this' (HSCS 5.11) and 'My environment has plenty of natural light and fresh air, and the lighting, ventilation and heating can be adjusted to meet my needs and wishes' (HSCS 5.21) .
