Requirement 1

1. By 26 June 2026, to ensure that people’s care and support needs are met effectively, the provider must ensure care is delivered in line with assessed needs and that any emerging changes and concerns are responded to appropriately. To do this, the provider must, at a minimum: a) ensure staff are competent in recognising changes or deterioration in people’s physical health, mental health, behaviour, or wellbeing b) ensure people’s day-to-day health and wellbeing needs, changes and concerns are recorded accurately and contemporaneously c) ensure changes or concerns are acted on without delay and escalated or referred appropriately to relevant others d) ensure personal plans and associated risk assessments are reviewed and updated promptly when people’s needs change e) ensure people receive dignified and respectful support, including personal care that maintains hygiene and skin integrity. This is to comply with Regulation 4(1)(a) of The Social Care and Social Work Improvement2. By 12 June 2026, to ensure people’s care and support needs are met effectively, the provider must ensure all medication is managed and administered safely. To do this, the provider must, at a minimum: a) ensure visit scheduling and timings consistently support safe medication administration, including adherence to prescribed dosing intervals b) ensure that where medication is administered, withheld, or not given, this is recorded accurately, clearly, and contemporaneously, including reasons for omission c) ensure clear, up-to-date medication guidance and protocols are in place and followed d) ensure staff who administer medication are trained, competent and confident in safe medication practice e) ensure regular and effective medication management oversight and auditing systems are in place f) ensure medication errors are identified, reviewed, and used to support learning and improvement. This is to comply with Regulation 4(1)(a) of The Social Care and Social Wo1. By 26 June 2026, to ensure people’s health, safety and wellbeing are protected, the provider must ensure effective leadership, quality assurance and management oversight of the service. To do this, the provider must, at a minimum: a) ensure effective quality assurance and audit systems are in place and used consistently to improve outcomes for people b) ensure protection concerns are identified, reported and escalated appropriately c) ensure statutory notifications include sufficient contextual information and are submitted timeously to relevant professional bodies, including the Care Inspectorate d) ensure the Service Improvement Plan (SIP) is regularly reviewed, updated and used as a live tool to drive improvement e) ensure people using the service and staff are meaningfully involved in service evaluation and improvement f) ensure leaders engage effectively with carers to support safe and consistent care delivery. This is to comply with Regulation 4(1)(a

Recommendation 1

1. To support people’s wellbeing and confidence in their care, the provider should improve how they communicate with people using the service, their families and professionals. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS), which state that: “My care and support meets my needs and is right for me” (HSCS 1.19); and “I get adequate and timely support and information when I need it” (HSCS 3.16).

Requirement 1

1. By 4 March 2026, the provider must ensure medication is managed and administered safely at all times to protect people’s health and wellbeing. To do this, the provider must, at a minimum: a) Implement individual protocols for all people prescribed medication to be taken as needed or as required (PRN). b) Ensure carers record the exact time of administration for every as needed (PRN) dose and check previous administration times before giving medication. c) Put in place effective systems for the safe storage and administration of controlled drugs. d) Provide staff with training and supervision to ensure they understand as needed (PRN) protocols, medication timings, and controlled drug procedures, and are competent in applying these safely. e) Implement a process for regular medication audits and spot checks to identify and address gaps promptly. This is to comply with section 8(1)(a) of the Health and Care1. By 4 March 2026, the provider must put in place effective rota scheduling and monitoring arrangements to ensure people receive their care at the right times and in line with their assessed needs. To do this, the provider must, at a minimum: a) Improve scheduling controls to prevent overlapping care times and reduce last-minute changes. b) Ensure consistent Electronic Call Monitoring (ECM) exception handling for late, early or unlogged calls, with recorded reasons and corrective actions. c) Communicate promptly with people when changes occur or carers are running late. d) Provide assurances that visit times allow for safe medication practice. e) Record and review all missed or cancelled visits, including reasons and actions taken. This is to comply with section 7(1)(a) of the Health and Care (Staffing) (Scotland) Act 2019 and Regulation 4(1)(a) of The Social Care and Social Work Improvement Scotla
