Orems Care Service Housing Support

1. To ensure care remains responsive and reflective of individuals’ changing needs, personal plans and risk assessments should be reviewed at least every six months. The service should implement a system to track and evidence six-monthly reviews, ensuring: • All care plans and risk assessments are reviewed and updated at least twice a year, or more frequently if needs change. • Review dates are clearly recorded, with planned future reviews scheduled and documented. • The involvement of guardians is recorded and updates are signed and dated by staff. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'I am fully involved in developing and reviewing my personal plan, which is always available to me' (HSCS 2.17).

To support a culture of continuous improvement and the learning and development of staff the service should: • Ensure supervision sessions include reflective practice, constructive feedback and, opportunities to discuss learning needs and development goals. • F acilitate team meetings which provide an opportunity to provide updates on best practice, enhance communication and share learning.
