To provide a service to adults with a learning disability and mental health problems and to older people living in their own homes and in the community.
Where the support is provided from more than one address the service must keep the Care Inspectorate informed of any changes to the addresses from which the service is provided.
The support will be provided by 2 staff teams as agreed in the variation to conditions granted on 08 March 2023.
