1. To provide a service to adults and older people with assessed housing support needs in their own homes.

2. The support will be provided by 2 staff teams (Technology Enabled Care Team (MECS) and the Reablement Team, which includes a small Crisis Care Team) as agreed in the notice granting variation dated 9 September 2016.
3. The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details, as it appears on this certificate.
