Recommendation 1

1. Care plans should be streamlined across all hub areas to ensure that all the required information is easy to find, either for individuals or family. This includes keeping each person's reviews up to date. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'I am fully involved in developing and reviewing my personal plan, which is always available to me' (HSCS 2.17).

Recommendation 2

1. Management should ensure that all hub areas are following the same processes for quality assurance, this includes using the same documents and systems to gather information. Having important information populated in the one document ensures a more standardised efficient process. This is to ensure that care and support is consistent with the Health and Social Care Standards, which state, “I benefit from a culture of continuous improvement, with the organisation having robust and transparent quality assurance processes” (HSCS 4.19).

Recommendation 3

2. The service should continue to develop their service development plan ensuring that robust internal audits and quality assurance systems are carried out and any actions identified are linked to the wider service development plan. The service development plan is a live document which should continuously be updated to evidence improvements. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: "I benefit from a culture of continuous improvement, with the organisation having robust and transparent quality assurance processes" (HSCS 4.19).
