Conditions of registration
To provide a service to people with learning disability mental health conditions acquired brain injuries and dementia living in their own homes and in the community.
The service will be provided by 4 staff teams located in the Scottish Borders Local Authority area.
The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details as it appears on this certificate.
