Any other conditions unique to the service:
1. To provide a service to adults with learning disabilities and physical disabilities living in their own homes at five locations and in the community.
2. The support will be provided by one staff team as agreed during the variation dated 3 December 2018 which is located at the service address. The Care Inspectorate should be informed of any subsequent changes.
3. The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details, as it appears on this certificate.
4. The registered manager is also the registered manager for Leonard Cheshire Services (Scotland) - Glasgow (CS2004075575).
