Requirements & recommendations
1. To ensure that people’s care and support needs are appropriately met, the provider should ensure personal plans contain all relevant health information with clear escalation protocols staff can access and follow if needed. This will ensure care and support is consistent with the Health and Social Care Standards which state: ‘My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met, as well as my wishes and choices.’ (HSCS 1.15).
Conditions of registration

1. To provide a service to adults with learning disabilities and physical disabilities living in their own homes at five locations.
2. The support will be provided by one staff team as agreed during the variation dated 3 December 2018, which is located at the service address. The Care Inspectorate should be informed of any subsequent changes.
3. The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details, as it appears on this certificate.
