Requirements & recommendations
1. The provider should ensure that all people supported have a personal plan that reflects current risks and agreed outcomes, needs and wishes. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: “My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met, as well as my wishes and choices" (HSCS 1.15).

2. The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details, as it appears on this certificate.
3. The support will be provided by 5 staff teams as agreed in the notice granting this variation dated 26 July 2016.
