1. To provide a service to adults with physical disabilities or learning disabilities or mental health problems or who are older adults living in their own homes in Aberdeen and Dundee.

2. The support will be provided by two staff teams located at the addresses agreed at 26 August 2019. The Care Inspectorate should be informed of any subsequent changes.
3. The provider shall inform service users and their representatives that the care service is registered with the Care Inspectorate and shall ensure that they are made aware of the name of the registered service with its contact details, as it appears on this certificate.
