Requirements & recommendations
1. To support positive outcomes for people the provider should ensure that personal plans detail how people are to be supported and these are reviewed six- monthly or when people’s needs change. This should include, but is not limited to ensuring when risks are identified, risk assessment details and risk reduction measures inform personal plans. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state: Health and Social Care Standards - My Support, My Life (2018). -2.17 I am fully involved in developing and reviewing my personal plan, which is always available to me. -3.21 I am protected from harm because people are alert and respond to signs of significant deterioration in my health and wellbeing, that I may be unhappy or may be at risk of harm.
Conditions of registration
The service will be provided to adults who are homeless or at risk of homelessness and also adults requiring support to sustain their homes due to learning disability and/or mental health issues and/or substance use in 6 locations and in their home and the community.
Where the support is provided from more than one address the service must keep the Care Inspectorate informed of any changes to the addresses from which the service is provided.
The service will be provided by 6 staff teams located in Edinburgh and Fife.
