Recommendation 1

1. To support people’s health and wellbeing and ensure effective communication with health professionals, the provider should improve the quality and consistency of health-related information recorded in personal plans. This should include ensuring that care plans contain up-to-date and detailed information about people's health conditions, diagnoses, and any relevant guidance from external professionals. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS), which state that: “My personal plan (sometimes referred to as a care plan) is right for me because it sets out how my needs will be met, as well as my wishes and choices” (HSCS 1.15) and “I am supported to make informed lifestyle choices affecting my health and wellbeing, and I am helped to use relevant screening and healthcare services” (HSCS 1.28).

Recommendation 2

1. To strengthen evidence-based practice and improve outcomes for people, the provider should ensure that regular self-evaluation of staffing arrangements is clearly linked to the evaluation of people’s personal outcomes. This should include using feedback from six-monthly care reviews and other outcome-focused evaluations to inform staffing decisions and deployment. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS), which state that: “My care and support is provided in a planned and safe way, including if there is an emergency or unexpected event” (HSCS 4.14) and “I experience high quality care and support because people have the necessary information and resources” (HSCS 4.27).
