To ensure people benefit from a culture of continuous improvement, the provider should ensure that all issues identified through quality assurance processes are formally recorded and incorporated into the overall service improvement plan. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: ‘I benefit from a culture of continuous improvement, with the organisation having robust and transparent quality assurance processes’ (HSCS 4.19).

To ensure people are kept safe, the provider should ensure that a clear process for recording all accident/incidents is implemented, that information is fully recorded to ensure compliance and meaningful analysis. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: ‘I benefit from a culture of continuous improvement, with the organisation having robust and transparent quality assurance processes’ (HSCS 4.19).

1. In order to achieve good outcomes, people's support plans and risk assessments should be personalised and reflect details of their specific needs. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'My personal plan( sometimes referred to as my care plan) is right for me because it sets out how my needs will be met, as well as my wishes and choices' (HSCS 1.15).
