1. To promote children and young people’s wellbeing, and to improve evaluation of outcomes the service should review it’s approach to care planning. This should include but is not limited to, a review of the use of the current care plan to ensure that SMART goals are in place for all young people so that progress can be consistently evaluated. This is to ensure that practice is consistent with the Health and Social Care Standards (HSCS) which state: “My personal plan (sometimes referred to as my care plan) is right for me because it sets out how my needs will be met as well as my wishes and choices” (HSCS 1.15)
