1. To support positive outcomes for children, the service should further develop self-evaluation procedures. This should include, but not be limited to, implementing a more structured approach to self-evaluation, such as using quality improvement frameworks and involving all staff in improvement discussions. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'I experience high quality care and support based on relevant evidence, guidance and best practice' (HSCS 4.11)

2. T o promote a culture of reflective practice which identifies and actions areas for improvement, the management team should ensure effective quality assurance processes are in place. This should include, but is not limited to, robust monitoring of service delivery including mealtimes, staff practice and interactions, transitions, and children's personal plans. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'I benefit from a culture of continuous improvement with the organisation having robust and transparent quality assurance processes' (HSCS 4.19).

3. To keep children safe, the manager should review safer recruitment procedures to ensure they are fully implemented and consistently followed. This should include, but not be limited to, obtaining two satisfactory written references before employment, maintaining accurate records of checks, and strengthening oversight and recording systems. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state that: 'I am confident that people who support and care for me have been appropriately and safely recruited' (HSCS 4.24).

1. To ensure children's health and wellbeing needs are fully met, the service should further develop personal plans. This should include, but not be limited to, ensuring all sections are completed, information clearly identifies how children's needs will be met, and plans are reviewed at least once every six months with parents. This will help ensure care and support are consistent with the Health and Social Care Standards (HSCS), which state: ‘My personal plan is right for me because it sets out how my needs will be met, as well as my wishes and choices’ (HSCS 1.15).
