Recommendation 1

1. To ensure that children and young people’s care needs are appropriately identified the service should improve it’s approach to care planning. This should include but is not limited to: a) Reviewing the content of care plans to ensure all key information is captured. b) Ensuring that care plans are reviewed regularly and updated appropriately when needs change. c) Ensuring that information from staff debriefs are used to inform care plans. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which state: 'My personal plan (sometimes referred to as my care plan) is right for me because it sets out how my needs will be met as well as my wishes and choices' (HSCS 1.15).
