1. To ensure people receive the care that is right for them and be confident in the people providing the care and support, the provider should ensure staff are consistent in following an agreed support plan. This should include but is not limited to, training specific to the individual’s needs. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS) which states that: ’My care and support meets my needs and is right for me’ (HSCS 1.19); and 'My care and support is consistent and stable because people work well together’ (HSCS 3.19).
1. To support people having confidence in the organisations providing care and support, the provider should clarify actions from reviews and continue to document communications with multi-disciplinary agencies. This should include but is not limited to, continued input from expertise within the organisation. This is to ensure that care and support is consistent with the Health and Social Care Standards (HSCS), which states that: ‘I benefit from different organisations working together and sharing information about me promptly where appropriate, and I understand how my privacy and confidentiality are respected’ (HSCS 4.18).
