In policing compliance with standards, direct observation of practice, direct interaction with patients, carers and staff, and audit of records should take priority over monitoring and audit of policies and protocols. The regulatory system should retain the capacity to undertake in-depth investigations where these appear to be required.

The government published "Hard Truths: the Journey to Putting Patients First" (Cm 8777) on 19 November 2013, responding to all 290 recommendations of the Francis Report. This followed an initial response "Patients First and Foremost" in March 2013. Key reforms included a new Chief Inspector of Hospitals, strengthened Care Quality Commission inspection regime, a statutory duty of candour, and the fit and proper person test for NHS directors. - 6 Feb 2026 · CQC CQC was fundamentally reformed after Francis: Sir Mike Richards appointed as first Chief Inspector of Hospitals (2013); new inspection methodology with large teams, specialist experts, unannounced visits; ratings system introduced (Outstanding/Good/Requires Improvement/Inadequate). However, the Penny Dash review (October 2024) found CQC had 'lost credibility' -- inspections dropped from 15,800 in 2019-20 to 6,700 in 2023-24; 500 unpublished inspection reports; registration backlogs. Structural and legislative changes largely delivered (duty of candour, FPPR, CQC overhaul, revalidation, Freedom to Speak Up Guardians). - 1 Apr 2016 · Legislation - Health and Social Care Act 2012 (Monitor reformed) Monitor merged with the Trust Development Authority to form NHS Improvement from 1 April 2016. NHS Improvement then merged with NHS England from 1 July 2022 under Health and Care Act 2022.
