Miss Dallison received fatal injuries in a road traffic collision on the B5013 at Willslock nr Uttoxeter on 11 May 2013. She was driving in a southerly direction and crashed on a bend. I enclose a photograph showing the approach to the bend in the direction that she travelled. I am not aware of other fatalities on this bend but at the inquest I heard that there had been a lot of collisions at the site. I was also told that on this stretch of road, works have been done on other locations but not recently here. It is also apparent when approaching the bend from the other direction there are warning signs (a bend sign and ‘Slow’ in the road) as well as a chevron. From where Miss Dallison was travelling there is a long straight where vehicles may accelerate and a bend and ‘Slow’ signs but no chevron. I should be very grateful if you could carry out a survey of the scene and see if further safety measures should be taken.
In my opinion action should be taken to prevent future deaths and I believe you have the power to take such action. You are under a duty to respond to this report within 56 days of the date of this report, namely by 18 November 2013. I may extend the period if requested to do so.
