Investigation and inquest
On 21ST October 2015 conducted an Inquest into the death of Alexander Stuart Hadley (born 14/9/1993) of Tyn Y FawnogDinonwic Gwnedd and Steffan Robert Vernon (born 20/10/1981) of which revealed matters that cause me concern.
Circumstances of the death
On 5"h June 2015 Steffan Robert Vernon and Alexander Stuart Hadley went swimming in a pool below the waterfall at Llanberis where they drowned because of the downward current caused by the waterfall, the coldness of the water and other matters that are not relevant to this referral.
Action should be taken
In my opinion action should be taken to prevent future deaths and believe you have power to take such action.
