Statutory public inquiry under the Inquiries Act 2005 into mental health care at Tees, Esk and Wear Valleys NHS Foundation Trust. The inquiry will establish what happened in serious failings in care, why they occurred and what needs to change to help ensure patients receive safe, high-quality care. His Honour Judge Roderick John Conwy Potter was appointed chair on 17 September 2026, and the inquiry formally started on 21 September 2026. The chair said his first priority was to meet and listen to people affected, whose input would help inform development of the terms of reference.
